How does alprostadil act?

Alprostadil has pronounced vasodilating properties and, therefore, when administered, it helps to fill the body of the penis with blood and form an erection.

How to take alprostadil?

Unlike Viagra, Cialis, and Levitra, this drug is injected directly into the penis by intracavernous injection into the back of the penis closer to the base. Alprostadil is administered 5-10 minutes before sexual intercourse, in the amount prescribed by the doctor. The duration of an erection should not exceed four hours.

Dosage and administration

Caverdzhekt is applied by direct intracavernous administration. The dose is selected individually by titration.

Intracavernous administration is performed under sterile conditions. The injection site is usually chosen on the dorsolateral surface of the proximal part of the penis. It is important to avoid getting the needle into the veins. The side and place of administration should be periodically changed.

The initial dose for erectile dysfunction of neurogenic origin after spinal cord injury is 1.25 μg, the second dose is 2.5 μg, and the third is 5 μg. Next, gradually increase the dose by 5 μg to achieve the optimal dose.

With erectile dysfunction, psychogenic, mixed, vascular etiology, the initial dose is 2.5 mcg. If there is a clinical response, the second dose is 5 mcg, if there is no answer, 7.5 mcg. Then the dose is gradually increased by 5-10 μg until the optimal dose is reached.

If a response to the administered dose is not observed, the next (higher) dose is administered within an hour. Upon receipt of a response, the interval between administration should be at least one day. The maximum frequency of injections is 1 time per day, 3 times a week.


A prolonged erection, priapism may be observed. The penis is aspirated under aseptic conditions (a “butterfly needle” is inserted into the cavernous bodies and 20-50 ml of blood is aspirated). If this does not provide the desired effect, an alpha-adrenergic agent is administered intracavernous, and if necessary, bypass surgery.
